Information We Collect
Through this website we collect the business contact and service details you submit in our request, quote, account and contact forms—such as company name, contact name, phone number, email address, service location, number of employees, requested date and time, testing or service type, DOT or non-DOT status, reason for testing and any operational notes you provide.
Customer Accounts
If you create a CrewAssure customer account, we also store your account email address, authentication credentials managed by our authentication provider, your company profile information, and the history of service requests submitted under your account. Account records are visible to you and to authorized CrewAssure personnel.
Information We Do Not Collect Here
This website is not used to collect employee medical information, laboratory results, or other sensitive testing records. Please do not submit that information through these forms or in general notes fields.
How We Use Information
Submitted information is used to respond to your request, confirm service details, provide quotes, complete required service documentation, manage your company account and communicate about billing. We do not sell submitted information and we do not use it for advertising.
Service Providers
We use third-party providers to host this website, store request and account records, and deliver email. These providers process information on our behalf and only as needed to operate the service.
Storage, Security and Retention
Requests submitted through this website are transmitted over HTTPS and stored in a restricted database that is not readable from the public website. Access controls limit each customer to their own records, and administrative access is limited to authorized CrewAssure personnel. Records are retained for as long as needed to service your account and to satisfy applicable recordkeeping requirements.
Your Requests
You may ask us to review, correct or delete the business contact and account information we hold about you. Some records associated with completed services may be subject to recordkeeping requirements and cannot be deleted on request.
